Holders of a Commercial Driver's License (CDL) have until Jan. 30, 2014 to verify their CDL’s valid medical certificate status with the Minnesota Department of Public Safety. First, CDL holders should click here to check their license’s medical certificate status. If the status is not “certified,” then submit a completed Commercial Driver License Medical Self-Certification form using the provided instructions. If no action is taken by Jan. 30, the statuses of affected CDLs will be downgraded to a Class D license in accordance with 49 CFR 383.71(h).
