Steps to Become a Provider:

Before becoming an active provider, you will need to submit your business information to MnDOT's Office of Freight and Commercial Vehicle Operations (OFCVO). Instructions and forms are in the PDF below:

Completion of online training is required within 90 days of the initial date of certification.
