Application for Utility Permit on Trunk Highway Right Of Way (form 2525) aka: Long form (PDF 180 KB)
MnDOT requires this form for the vast majority of utility placements and relocations. Utility owners use this form to request permission to place, construct, and reconstruct utilities within trunk highway right of way, whether longitudinal, oblique, or perpendicular to the centerline of the highway.
Application for Installation of Utilities or Miscellaneous Work on Trunk Highway Right Of Way (form 1723) aka: Short form (PDF 366 KB)
MnDOT Districts require utility owners to submit this form for minor work, such as installation of utility service connections that do not cross or parallel the roadway within the trunk highway right of way. This form is also used to:
- Maintain utility facilities;
- Install miscellaneous guy wires and anchors;
- Place temporary obstructions on the right of way;
- Perform temporary relocations of a more minor nature to accommodate a construction project;
- Vegetation removal

MnDOT Districts require this form for all requests for access on a trunk highway.
