Title VI discrimination complaint procedure

MnDOT is committed to ensuring that no person in the State of Minnesota, on the basis of race, color, national origin, sex, age, disability, or income status, is excluded from participation in, denied benefits of, or otherwise is subjected to discrimination under any and all programs, services, or activities administered by the department, its recipients, subrecipients, and contractors. Additionally, MnDOT is committed to ensuring that its programs incorporate access for people with Limited English Proficiency.

Who can file a discrimination complaint

Please complete the MnDOT discrimination complaint form if you feel that you have experienced discrimination by MnDOT, its sub-recipients, consultants, or contractors on the basis of race, color, national origin, sex, age, income status, religion, or disability, in violation of the Title VI Act of 1964 and related statutes.


Complaints shall be a written or electronic statement signed by the complainant or the complainant’s representative. All sections of the MnDOT discrimination complaint form must be completed prior to submission.

Time limit for filing a discrimination complaint

The complaint must be filed within 180 calendar days from the date of the alleged discriminatory incident.

The discrimination complaint form is available in English, Spanish, Hmong, and Somali. Requests for alternative formats or additional languages of the complaint form should be directed to ADArequest.dot@state.mn.us or by phone at 651-366-4718 or TTY 800-627-3529.

If you need assistance completing the form, please call or email the MnDOT Office of Civil Rights. Allegations of discrimination received by fax or email will be acknowledged and processed. Allegations received orally by phone or other means will be produced in writing and provided to the complainant for approval.
